Ejaculating into another character's vagina has a chance to impregnate them, leading to pregnancy and eventually birth. Slimes can be impregnated when taking cum into any orifice.

In addition to becoming pregnant, characters can also host eggs in various orifices. A pregnant character can use the egg-laying sex action when penetrating an appropriate, unoccupied orifice with a penis or clitoris with the OVIPOSITOR modifier (or a demonic ovipositor tail). Eggs laid through the mouth or anus incubate in the stomach, while eggs laid into the vagina incubate in the womb and block pregnancy until they are laid. If the nipples on a character's breasts or crotch-breasts are penetrable, they can incubate eggs.

The "Risk of Pregnancy" status effect is applied whenever a character has cum in their vagina (or for slimes, cum in any orifice) and lasts between 4-8 hours. In the background, a pregnancy roll is performed at the same time in the background (see the Impregnation Check section below). A pregnancy roll is also performed when the effect ends, which may impregnate the character if there is any cum remaining in their vagina. If a character has the effect but is not pregnant, the pregnancy check can be "re-rolled" by using the Mother's Milk item, or by saving with the effect and loading the save.

The "Pregnant", "Heavily Pregnant" stages occur next after the "Risk of Pregnancy" status effect ends, if an impregnation check succeeds. Each stage lasts half a week by default (varying by about 12 hours), but total pregnancy duration in weeks can be set in Content Options>Gameplay. When the "Pregnant" status effect ends, the character's breasts may grow. When the "Heavily Pregnant" status effect ends, a character may begin lactating, or existing lactation may increase. Options to control whether pregnancy causes breast growth or lactation, and to what limit, are found in Content Options>Bodies.

Each egg incubation stage is tracked separately for each area (Stomach, Vagina, Breasts & Crotch-Breasts). The "Egg Incubation" and "Advanced Egg Incubation" stages last between 72-84 hours.

The "Ready for Birthing" and "Completed Egg Incubation" stages last indefinitely, until the character births or lays the eggs.

The Mother's Milk item advances pregnancy and egg incubation stages, and can be used on the player as well as pregnant NPCs. Using it in the "Risk of Pregnancy" stage ends the status effect, which is immediately re-added if the character still has cum in their vagina.

Elementals and Dolls can not impregnate others or become pregnant themselves

Visibly Pregnant

Characters that are already visible pregnant will get an Already Pregnant response, for example

You feel cum deep in your pussy but because you're already pregnant, you don't have to worry about it!

Incubating Eggs

Characters that are incubating eggs in their vagina will get an Already Pregnant w/ Eggs response, for example

You feel cum deep in your pussy but because your womb is already filled with eggs, you don't have to worry about it!

Phase 1 - Can be pregnant

Father has a Virility attribute greater than 0 OR doesn't have the Firing Blanks perk
Mother has a Fertility attribute greater than 0 OR doesn't have the Barren perk, additionally doesn't have the Menopause status effect
Mother character type has isAbleToBeImpregnated flag set to true

If any of these checks fail then chance is set to 0% and the next phase will be skipped

Phase 2 - Pregnancy Chance

Base Chance is 10%

Father's Virility attribute is divided by 2, then added to Pregnancy Chance
Mother's Fertility attribute is divided by 2, then added to Pregnancy Chance

Both Virility & Fertility are soft-capped at -100 & 100, so the pregnancy chance will now be somewhere between -90% and 110%

Phase 3 - Risk of Pregnancy

If force pregnancy on next check (currently only used in Bad Ends) then set pregnancy chance to 100% and clear force pregnancy flag

At this stage the game will give a pregnancy possibility response, depending on the pregnancy chance

Unlike the auto-fail condition Visibly Pregnant which only checks if the Mother is showing (has progressed past "Risk of Pregnancy" status effect) this stage checks if the character is already pregnant, if they are then all remaining checks and actions are skipped

Next if the Mother doesn't already have the "Risk of Pregnancy" status effect, they gain it for 5 to 8 hours

If RNG is below or equal to the pregnancy chance then the Mother becomes pregnant, if not all remaining checks and actions are skipped

Phase 4 - Offspring Generation

Litter size is determined by the following rules

If Mother is a Slime then use Slime
If Mother's vagina is not Human then use Mother's vagina race
If Mother's vagina is None then use Mother race
If Mother's vagina is Human then use Father's penis race
If Father's Penis is None then use Father's race

minimumNumberOfChildren and maximumNumberOfChildren are assigned based on the race determined by this check

If Father has Seeder perk active multiply maximumNumberOfChildren by 2

numberOfChildren is now assigned as minimumNumberOfChildren plus a random number between 0 & maximumNumberOfChildren minus minimumNumberOfChildren

If Mother has Broodmother pill status effect multiply numberOfChildren by 2
If Father has Broodmother pill status effect multiply numberOfChildren by 2

The game will now create a number of offspring seeds equal to the numberOfChildren and add them to the same Litter

Giving Birth / Laying Eggs

Player

The Player currently has to return to the Mansion and use Lilaya's Birthing Room to give birth. They can lay eggs if they meet the following conditions

Are in a neutral scene
Location allows sex
Can access the area that is incubating eggs
If the location is populated, the Player needs the Exhibitionist fetish

They can hold onto their pregnancy/incubation indefinitely with no penalties beyond the status effects from their pregnancy/incubation(s)

NPCs

All NPCs will give birth/lay eggs 12 hours after they reach stage 3 as soon as the player is not occupying the same tile
